Orthobiologics in Shoulder Surgery: What’s the Hype?

Orthobiologics refer to substances derived from natural sources that help musculoskeletal injuries heal more efficiently. In shoulder surgery, orthobiologics such as PRP, bone marrow aspirate concentrate (BMAC), and collagen scaffolds are used to augment rotator cuff repairs, labral repairs, and cartilage regeneration procedures.

These materials promote tissue regeneration and reduce inflammation. Their use is particularly beneficial in chronic or large tendon tears where healing potential is otherwise low. Some products are applied during surgery, while others may be injected post-operatively to support healing.

Clinical evidence is growing but still limited regarding their long-term effectiveness. Many surgeons use orthobiologics selectively, often in younger patients or those undergoing revision surgeries. Cost and availability can also be limiting factors.

While not a substitute for good surgical technique and rehabilitation, orthobiologics are an exciting adjunct that may improve outcomes, especially in complex or high-risk cases. Patients should consult their surgeon about the benefits, risks, and current research before opting in.
